    Forms & Fees

    Intake Paperwork

    The Notice of Privacy Practices, Client Information & Informed Consent, Informed Consent: Financial Policies, Virtual Therapy, and Notice of Good Faith Estimate are included here for your information. However, when you register through my Client Portal, you will be prompted to read all of that information, consent to treatment, and complete a form about your health history and current symptoms. So, when you schedule, there is no need to print anything out nor to arrive early to fill out paperwork. If you have any questions throughout the process feel free to reach out to my assistant or make note and bring it up in the first session.

    Rates

    • $145.00 per 55 minute intake (1st session only)
    • $130.00 per 55 minute individual session
    • $170.00 per 85 minute couples/family session

    Insurance

    Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for psychotherapy services. Please note that you must meet “medical necessity” criteria, meaning you truly have a mental health diagnosis, for insurance to pay for services. If you have not already received such a diagnosis the presence of one can be determined during our initial sessions.

    I’d recommend asking these questions to your insurance provider to help determine your benefits:

    • Does my health insurance plan include mental health benefits? Which insurance company provides those benefits? (note, sometimes it’s different than your health insurance)
    • (If seeking something other than individual) Does my mental health plan cover couples, family, or group counseling?
    • Do I have a deductible? If so, what is it and have I met it yet?
    • What is my co-pay or co-insurance amount per session?

    I accept the following insurances:

    • Aetna
    • Anthem/BCBS
    • Cigna
    • Optum/United Healthcare

    I am also able to provide you with the necessary forms if you would like to use your Out-of-Network benefits with any insurances I do not accept to help defray the cost.

    Employee Assistance Programs (EAP)

    EAPs are a different product than insurance and often authorize a few free sessions. If unsure if you have this benefit, check with your boss or human resources department.

    Payment

    I accept cash, check and all major credit cards, including HSA, as forms of payment. Electronic payments can be accepted via my Client Portal.

    Cancellation Policy

    If you are unable to attend a session, please make sure you cancel/reschedule at least 24 hours beforehand by logging into your account on the Client Portal or by calling. Otherwise, you may be charged the $80.00 no show/late cancellation fee.
